Billing & Insurance

Proof of Insurance

Please bring your insurance card(s) and drivers’ license with you to every appointment. It is your responsibility to inform the front desk when the cause for treatment should be billed to someone other than your regular primary insurance.

Payment Information

Piedmont Vista accepts cash, personal checks, debit and credit cards. Any co-pay required by the insurance company must be paid at the time of check-in.

All Medicare patients will be required to pay their 20% co-pay at the time of check out unless proof of a secondary insurance policy is presented.

Self pay patients should be prepared to pay $200 initial payment prior to the visit.

Insurance

We will bill any insurance company on your behalf with whom we participate under contract. If we do not contract with your insurance company, we will collect payment in full at the time of service and will file your insurance as a courtesy to you. While we do provide benefit information on allergy testing and surgery services as a courtesy, it is ultimately your responsibility as the policy holder to be aware of your insurance benefits and requirements prior to any scheduled visit.

Financial Assistance

Our office treats patients regardless of financial status. If you have no insurance, have maximized your benefits or have a high deductible, ask to speak with the Practice Manager regarding special financial assistance programs available to you.
