Preparing For Your Visit

Piedmont Vista has a computer-based medical records system that enables us to offer home registration to our patients. By using our secure web portal at piedmontvista.com., with the password, you may plan to arrive at your scheduled appointment time. At the website, you will enter your personal information; past medical, family, and social history; and important questions about your health. Please answer these questions thoughtfully, as they form the basis for your future treatment at Piedmont Vista. Alternatively, If you prefer not to register via web portal, we will send you a packet of information to complete at home and bring with you to your appointment. You will be asked to complete one additional form once you arrive in the office.

If your preference is to register at the office, plan to arrive for your appointment 15 minutes prior to your scheduled time and please be sure to bring a complete list of your current medications with you, including over-the-counter and herbal medicines. Whichever way you choose to register, our staff will be glad to assist you, by phone or in the office.

We participate with most insurance plans, so it is important to bring your insurance card and valid identification to the visit. We will also expect to collect your co-pay or payment in full (if you aren’t insured) at the time of service.

To assist us in serving you better, we will want to take a facial photo of you at the computer kiosk in the reception area just prior to your appointment.
